Giveaway Terms & Conditions
Eligibility: This giveaway is open to legal residents of Middle Tennessee who are at least 18 years of age at the time of entry.
Entry Methods: To enter, participants may fill out the above form on the Medical House Calls website. No purchase is necessary to enter. Entry is limited to one submission per person. Duplicate or multiple entries from the same individual will be disqualified.
Prize Information: One winner will receive a free 12-month membership to the Concierge Urgent Care Gold plan from Medical House Calls, valued at $5,000. This membership covers all services within the Gold plan but is non-transferable, cannot be redeemed for cash, and may not be substituted for other services or benefits. Additional services outside of the Gold plan may incur extra fees.
Drawing Details: The winner will be selected by a random drawing on December 31, 2024, at 12:00 PM CST. The drawing will be conducted under the supervision of Medical House Calls representatives, and the decision will be final and binding.
Winner Notification: The winner will be notified via email within 3 business days after the drawing. To claim the prize, the winner must respond within 7 days of notification. Failure to respond or to provide any required information may result in disqualification, and an alternate winner may be selected.
